To order a Foundation Medicine service for a patient, please follow these steps:

Step 1:Gather all the required documents
Please make sure the following forms are completed or discussed with the patient:

Step 2: Submit your Requisition
Order a Foundation Medicine service through our Canadian Foundation Medicine Ordering Portal. Please, always attach the following documents with your order
Pathology Report
Foundation Navigate Form

Step 3 : Once your requisition has been successfully submitted
Our Team will contact the payor to collect the payment prior to testing

Step 4 : Specimen Retrieval and Shipment : For tissue samples
Our team will provide the collection kit, instructions, Test Requisition Form (TRF), and shipping labels to the pathology lab for sending the sample to Foundation Medicine.

Our team will contact the patient to schedule the blood draw and provide the clinic with the collection kit, instructions, Test Requisition Form (TRF), and shipping labels to send the sample to Foundation Medicine.

Once the specimen has been delivered to Foundation Medicine, a specialized team will begin the testing process

Step 6 : Medical Report
A genomic profiling report is generated and provided to the ordering physician. Healthcare professionals use the profiling report to assist in creating treatment plans for their patients.
